This illumination device comprises: a light guide plate (11), one end surface of which is formed as an incident surface (11a), and which is bent at a bent section (11c) between the incident surface (11a) and a tip end surface (11b) on the side opposite the incident surface (11a); and a light source (12) which is arranged so as to face the incident surface (11a). The light guide plate (11) comprises: a plurality of first prisms (13) which are formed on a first surface (11d), of the light guide plate, between the incident surface and the bent section, which reflect light emitted by the light source into the light guide plate from the incident surface, and which output light from a second surface (11f) on the reverse side to the first surface (11d); and a plurality of second prisms (14) which are formed on a third surface (11e), of the light guide plate, between the tip end surface and the bent section, which reflect light emitted by the light source into the light guide plate from the incident surface, and which output light from a fourth surface (11g) on the reverse side to the third surface (11e).
