A needle guide and a puncture device. The needle guide comprises a rotary plate (4), a needle groove block (5), a positioning seat (3) and a ring body (1); a needle groove (54) is provided on the surface of the needle groove block (5), and the plane where the needle groove (54) is located is used for fitting with the surface of the rotary plate (4) so as to form a needle guiding channel; the positioning seat (3) is connected to the ring body (1), a positioning structure is provided between the positioning seat (3) and the needle groove block (5), and the positioning structure is located outside the ring shape of the ring body (1); and the first end of the rotary plate (4) is rotatably connected to the positioning seat (3), and the second end of the rotary plate can be fixed at a plurality of positions, so that the needle guiding channel can be located at a plurality of angles. Multi-angle adjustment of the needle guide can be achieved by inserting and pulling out the needle groove block (5) and adjusting the rotary plate (4), providing the needle guide with the advantages of simple structure and being convenient to operate.
