Provided are an optical system that can improve the usage efficiency of image light from a display element, and an image display device. A light guide member (5) of the optical system comprises an emission extension region (53) having a diffractive structure that splits, in a first propagation direction intersecting a thickness direction of a main body part (50), image light (L1B) that enters inside the main body part (50) from an incident region (51) and propagates in the first propagation direction into a plurality of image light (L1C) propagating in a second propagation direction intersecting the first propagation direction, and causes the plurality of image light to be emitted from the light guide member (5). In a case assuming that, in at least a portion of the emission extension region (53), the refractive indices of the medium are equal between the incident side and emission side of the image light with respect to the emission extension region (53), an emission angle θ_out of image light (LIC) emitted at the highest diffraction efficiency from the emission extension region (53) in a prescribed plane (P1) including the normal (N) of the emission extension region (53) is 15-45°, and the incident angle θ_in of image light (LIB) incident on the emission extension region (53) in the prescribed plane (P1) is at least 14° greater than the emission angle θ_out.
