A planar illumination device (1) according to an embodiment comprises a substrate (2), a first optical element (5a), a second optical element (6a), a third optical element (7a), a pair of fourth and fifth optical elements (6b, 7b), and a sixth optical element (10a). A plurality of light sources (3) are two-dimensionally disposed on the substrate (2). The first optical element (5a) is disposed on the emission side of the plurality of light sources (3), and collects light emitted from the plurality of light sources (3). The second optical element (6a) is disposed on the emission side of the first optical element (5a), and tilts the distribution of the light collected by the first optical element (5a) with respect to a first axial direction within an emission surface. The third optical element (7a) is disposed on the emission side of the second optical element (6a), and extends the light tilted by the second optical element (6a) in the first axial direction. The pair of fourth and fifth optical elements (6b, 7b) are disposed on the emission side of the plurality of light sources (3), and extend light in a second axial direction orthogonal to the first axial direction within the emission surface. The sixth optical element (10a) is substituted for the second optical element (6a) and the third optical element (7a) by a combination of the second optical element (6a) and the third optical element (7a).
