A position detection method and device, and a radiotherapy system, belonging to the field of radiotherapy. Said system comprises a treatment bed (01), a positioning device (02), an optical tracking system (03) and an upper computer (04); the positioning device (02) is provided on the treatment bed (01), and at least one marking reference point (A) is provided on the positioning device (02); the optical tracking system (03) is provided above the treatment bed (01) and is used for detecting the relative position of a marking point provided at an affected part relative to the marking reference point (A), determining the deviation of the relative position from a reference position, and sending the deviation to the upper computer (04); and the upper computer (04) is used for determining whether it is necessary to adjust the position of the treatment bed (01) on the basis of the deviation and a deviation range. Said system can avoid the influence of patient movement on the treatment accuracy, and can prevent a treatment beam from damaging the normal tissue of the patient.
