A spacer (40) is placed between side plate portions (33) of adjacent forms (30) when a plurality of the forms (30) having dam plate portions (32) and side plate portions (33) are installed in a row arrangement. One side of each dam plate portion (32) serves as a concrete placing surface (31). The side plate portions (33) of the form (30) are bent at right angles, from both side edges of the dam plate portion (32), toward the opposite side of the concrete placing surface (31) of the dam plate portion (32). In a concrete form structure (50), the spacers (40) are placed between the side plate portions (33) of the adjacent forms (30), and separators (22) are inserted through insertion holes of the spacers (40). Using such spacers (40) enables application of metal members used for conventional plywood forms composed of plywood to the concrete form structure without providing holes and grooves in the forms (30) for the metal members.
