Abstract:
PURPOSE:To eliminate the transfer error and make clerical work on insurance easy and rapid by placing the form of clerical work for medical treatment on a cover for the original to be copied and by copying the receiver's name, etc., described said form and the insurance medical treatment identification mark on another said form. CONSTITUTION:At the reception etc., of a medical organization, the form of clerical work for insurance medical treatment form is entered with prescribed matters on the column 11 describing the patient's name, etc., the column 12 describing the insurer number, marks and the codes and numbers of the insurance certificate and notebook, and the column 13 describing the public expense receiver and service receiving number, based upon the insurance certificate submitted by the patient. Form A is set to the paper feeder of the copier. Next, the insurance certificate cover E is set to the original platen of the copier, and the insurance certificate submitted by the receiver is placed over the transparent window portion 51 of the cover E. Then the copier is operated. By this arrangement, the insurance certificate is copied in the copy column 14 of the form A. After being clerically processed, Form A is passed to the doctor, and while diagnosing a patient, he enters prescribed matters, such as the patient's name, previous illness, cause of the desease, main symptom, prescription, operation, etc., in the column 15.
