PURPOSE: To print a label on the disk-shaped flat surface over the entire area on the upper side of a protective film by packing a resin into annular groove parts transferred at the time of molding a disk substrate.

CONSTITUTION: The disk 1A is constituted with the disk substrate 2, a reflection film 3 which is formed as a thin film on the information signal surface 2c side of the disk substrate 2, the protective film 4 which is formed as a thin film on the reflection film 3 and the label 6A which is printed on the protective film 4. The annular groove part 4b is transferred in a recessed shape on the information signal surface 2c side by an annular collar part of a collared retainer for pressing the central part of a stamper at the time of molding. The resin 7 is packed into the annular groove parts 2b, 3b, 4b to form the annular flat surface 7a during any process after the molding of the disk substrate 2. After the thin film of the protective film 4 is formed, the label 6A is printed to the desired position on the disk-shaped flat surface 4A over the entire area on the upper side of the protective film 4 exclusive of a center hole.
