A 1st disk 11 having 1st and 2nd information recording faces and a 2nd disk 12 having 3rd and 4th information recording faces are stuck together with an adhesive 8 or the like. The 1st disk 11 has a structure formed by successively laminating a semi-transmissive 1st reflecting layer 4, a 1st protective film 13, a light transmissive sheet 14, a transfer layer (UV-curing resin layer) 15 having the 2nd information recording face, a 2nd reflecting layer 6 and a 2nd protective film 16 on the 1st information recording face of a 1st light transmissive substrate 3. The 1st protective film 13, the light transmissive sheet 14 and the transfer layer 15 constitute a middle layer 17. The 2nd disk 12 is similar in structure to the 1st disk 11.
