PURPOSE: To obtain stabilized recording sensitivity by pressing a stamper on the recording layer on a transparent resin substrate and forming a preformat pattern.

CONSTITUTION: A recording layer 2 is formed on a transparent resin substrate 1, a stamper 7 having projecting part 18 corresponding to the preformat pattern is pressed against the surface of the recording layer 2 to force the projecting part 18 into the substrate 1 to a specified depth, and the part of the recording layer 2 is plastic-deformed to form a striped track pattern 3. The stamper 7 is provided with a heating part 8. The track pattern 3 is formed at the deformed part of the recording layer 2 and a recording part 4 at the undeformed part. A card substrate 6 is stuck on the pattern 3 and the recording part 4 through an adhesive layer 5 to obtain an optical card.
