To easily and rapidly form a spacer layer made uniform over the entire part and to improve productivity by rotating both of a substrate and a rotary stamper while crushing a UV curing resin by bringing the substrate and the rotary stamper into pressurized contact with each other.

The liquid UV curing resin 103a is supplied in the state of rotating the stamper 107 at a low speed. The polycarbonate substrate 101 and the stamper 107 are rotated at a high speed at the time of crushing this resin between the polycarbonate substrate 101 and the stamper 107. The UV curing resin 103a is conveniently spread between the polycarbonate substrate 101 and the stamper 107 by the centrifugal force generated by this rotation. Since the spacer layer 103 is cured by applying UV rays in the form of spot light thereto, the spacer layer is eventually gradually solidified while the film thickness is controlled. The spacer layer which has the good distribution accuracy and is made uniform as a whole is easily and rapidly formed.
