PURPOSE: To provide an adhesive memorandum paper which indicates a memorandum or a message three-dimensionally with an appealing effect without readily exposing the memorandum or the message, and can be easily used.

CONSTITUTION: This adhesive form for memorandum comprises a memorandum entry part 3 provided on the surface of the main body 2 of the form, an adhesive part 4 which can be reattached to and reseparated from a desired surface along the side edge of the rear surface end part of the main body 2, and a binding surface 5 equal to the size of part of the desired surface of the adhesive part 4, along the side edge of the end part on the opposite side to the adhesive part 4, on the main body 2 of the form for memorandum. In addition, the surface of the main body 2 is turned inside, and the binding surface 5 is lapped on and bound together with part of the adhesive part 4, and the surface to be attached can be attached by the adhesive part 4 which is not yet lapped. Besides, this adhesive form for memorandum has a memorandum display part on the rear surface of the main body 2.
