To provide a delivery slip, which is produced by forming an invoice and a receipt on one paper and in which a separating paper is made to be easily separable from a pasting slip paper to be pasted to a distributing article.

A paper stock, to the self-adhesive coated surface of a surface layer paper of which the separating paper is separably bonded, is employed so as to separate the inside separating paper through cuts 15 and 16, which are formed at the middle part and near either one end of the separating paper 13 of the paper stock so deep as to reach the self-adhesive of the surface layer paper, and then inseparably bond another paper 17 to the self-adhesive coated surface with exposed self-adhesive. Wavy slits 22 and 23 having the depth of cut reaching the self-adhesive of the surface layer paper are provided in portions forming the neighborhood of the boundary parts between the respective separating papers 13 and the paper 17. Along the two wavy slits 22 and 23, perforations 4 and 5 are formed. The area, to which the paper 17 is bonded, of the self-adhesive coated surface of the surface layer paper is used as the receipt 3, while the other area is used as the invoice 2 and a pasting blank form 6. The delivery slip is pasted to the distributing article after the separation of the separating paper 13 therefrom. The receipt 3 is recovered from the destination of the slip.
