PURPOSE: To unify the numbers between an application form and an individual form and to improve the efficiency of business work, by reading a sheet number out of a sheet containing data, recognizing and storing the sheet number to a number memory means and reading out this stored number to give the number to an individual form as an individual form number.

CONSTITUTION: A reading part 1 is provided to an individual form producer to read the contents written on an application form 10, and a control part 5 is connected to the part 1. The number of the form 10 read by the part 1 is recognized at a number recognizing part 2a of a recognizing part 2 and stored at a number memory part 3a of a memory part 3. The prescribed form and data are read out of a font generating part 6 to produce an individual form for each delivery destination and stored to the part 3a. Then the number of the form 10 stored in the part 3a is given to the individual form stored in the part 3. A font signal of the part 6 is applied to a printer 7, and the individual form is printed based on said font signal and the data of the part 3. Thus the efficiency is improved for business work with forms.
