On a flexible substrate 30 having a wiring pattern 21, a block 40 positioning optical fibers 2a and 2b at positioning grooves (first positioning parts) 41a and 41b and having a projection 42 (second positioning part) for positioning an optical component 50 for aligning the optical fibers 2a and 2b and a light emitting element, and the optical component 50 having an optical semiconductor element are mounted. After bending the flexible substrate 20 from a B-B line, when the substrate 53 of the optical component 50 is positioned to the projection 42 of the block 40, the respective cores of the optical fibers 2a and 2b and the light emitting parts 510 and 520 of first and second light emitting elements 51 and 52 are aligned. After the alignment, the adhesive application area 25 of the bend part 24 of the flexible substrate 20 is bonded to the side face 43 of the block 40.
