To obtain a composition for prevention or treatment for infection having potent antimicrobial activity and weak toxicity to kidney by including a specific 1-methylcarbapenem compound as an active ingredient.
This composition includes a 1-methylcarbapenem compound of formula I (R1 and R2 are each H or a lower alkyl; R3 is H, a cycloalkyl or the like; however, the case that all the R1, R2 and R3 are H is excluded), its salt or its derivative as an active ingredient. As the compound of formula I, (1R,5S,6S)-6-[(1R)-1-hydroxyethyl]-methyl-2-[(2S,4S)-2-[(3R)-3- methylaminomethylpyrrolidin-1-ylcarbonyl]pyrrolidin-4-ylthio]-1-carbapen-2-em-3- carboxylic acid or the like is favorable. The compound of formula I is obtained by, for example, reacting a compound of formula II (RL is an eliminable group; R5 is a carboxyl-protecting group) with a compound of formula III (R1p is R1 or an amino-protecting group; R2p and R3p are each an amino-protecting group or the like).
