PURPOSE: To easily prepare an exact medical format by providing a pair of an IC card for a doctor and an IC card for a patient, and using an IC card connected with a printer.

CONSTITUTION: This device is provided with an IC card reader 3 and a pair of IC cards equipped with an IC card 8 for a doctor and IC card 9 for a patient for easily printing a medical format. According to the principle of this device, the IC card 8 for a doctor is introduced to the reader 3 immediately after a consultation hour starts, and the reader 3 is set by this IC card 8. Next, when the IC card 9 for a patient is introduced, the printing of a format having display related with a doctor who issues prescription and a patient who receives it can be started at a proper place.
