PURPOSE: To solve inconvenience for confirming the position of a pad sheet by together retaining both a removable sheet and a sheet for recording of each pair in the leaf edge parts.

CONSTITUTION: A removable sheet 1 and a sheet 2 for recording are respectively provided with leaf edge parts 10, 11 which are parallel to a back edge 6 and arranged with an interval in the cross direction from the back edge 6 and extended in the longitudinal direction. When sheets are piled up and closed by binding rings 5, the leaf edge parts 10, 11 of a set of sheets are overlapped and the leaf edges 7 of the business form for recording or a pad are formed. Breadth from a leaf edge 7 extending in the longitudinal direction of the removable sheet 1 is shorter than breadth from the leaf edge 7 of the sheet 2 for recording. The leaf edge parts 10, 11 of the sheets 1, 2 are together retained at every pair. Each pair of sheets can be integrally rotated similarly to a sheet of paper while keeping the binding rings 5 as axes. A pad sheet 25A is formed as a part of a cover 25B.
