Predetermined characters, phrases, items and columns are previously printed on sheets of cut form sheets 2 to provide an aid of filling-in the columns. The sheets 2 of necessary number are bound as one cut form set 1. In this case, at the part on a left side as one faces it, the sheets 2 are adhered to each other via paste parts 3 of a lightly releasable adhesive. As a preferred example of the adhesive to be used, an emulsion of polybutadiene can be used. In the case of using such a cut form set 1, a user fills in as prescribed, and then the sheets 2 adhered at the parts 3 are turned over one by one from above, and released.
