To provide a delivery slip which is two-ply delivery slip consisting of the lower layer slip paper and upper one, wherein a part or design not be altered individually is printed beforehand, which is easy to be used for circulation enterprisers, or users of the delivery slip, and is easy to be checked for customers, and wherein all the information can be checked efficiently at a first glance, with cost reduction and environmental protection.

The delivery slip is formed by stacking the upper slip paper on the lower slip paper, and the surface of lower slip paper is provided with a wrapping paper information column. The surface of the upper slip paper is provided with a report slip, a delivery note and a copy of delivery note. The report slip, delivery note and its copy are provided with at least recipient information and sender information. The delivery note and its copy are set so that they may be freely separated from the upper slip paper. The delivery slip is composed so that the wrapping paper information column shown on the lower layer slip paper may be visible in the separation marks of the delivery note and its copy.
