To provide a simplified envelope type form not having copy paper pieces for use in printing confidential information.

The envelope type form is an envelope made by overlapping a first paper piece and a second paper piece having information including address information printed therein and bonding their peripheral portions to each other. The inside of the envelope is provided with a copying structure, and two or more pattern-printed portions with a colored ink, emboss forming portions and printing portions with an ink including peeling-off constituents are combined to each other within a confidence information printing region arranged at a part of the surface in the first paper piece having information including address information printed therein.
