To perform an application work at a low cost without the necessity to fit the anchoring face or the bottom face of a block body to different angles of skew by installing a bearing plate formed of a specified angle of inclination given to an anchoring plate, on an anchor introduction part on the upper face of a form in such a way that the anchoring plate is orthogonal with the driving angle of an anchor.

A bearing plate 100 is structured of an anchoring plate 110 and supporting parts 120-123 for supporting the anchoring plate 110 which are formed on the peripheral edge part of the anchoring plate 110. In the center of the anchoring plate 110, an anchor introduction hole 111 is opened and near the end part of the opposite side to the anchoring plate 110 of the supporting plates 120-123, split bottom plates 130, 131 are fitted with an anchor introduction slit 132 interposed in the center, leaving the terminal fitting parts of the supporting parts 120-123 as they are. Thus the form for a concrete block is constructed so that the anchoring plate 110 is orthogonal with the driving angle of an anchor by setting the supporting parts 120-123 at partially different elevations from each other, in such a state that the bearing plate 100 is mounted on the upper face of the anchoring plate 110.
