PURPOSE: To prevent the occurrence of flaws in an optical storage area due to scratching and dust by providing a cover member for housing the storage disk in a hollow formed above a substrate member and an information holding layer to be read out mechanically and visually.

CONSTITUTION: A frame 36 on a substrate 12 is provided with the information holding layer 35 which is freely attachable and detachable, and personal identification is performed by mechanically and visually reading out personal identification information of the layer 35. On the other hand, the optical data storage disk 20 recorded with its personal data in details such as medical data, etc., is freely attachably and detachably located in the hollow formed by the cover member 14 equipped with a freely movable shutter member 34 for covering a middle boss opening 22 and a data access opening 28, and is stored with the personal identification information and personal information in different areas respectively. Thus, by the personal identification information to be frequently used, the occurrence of flaws due to scratching and dust in the optical storage area is prevented.
