Abstract:
PROBLEM TO BE SOLVED: To provide a my-number(R) registration slip and a check slip in order that an organization registers and manages a confidential number of a constituent member or the like onto a data base and to provide a my-number registration/confirmation method using the slips.SOLUTION: A confidential number registration slip of a confidential number registration slip and form pair type makes confirmation of two sets of information of A. (a second temporary registration number or a first temporary registration number and a confidential number described on a confidential number description card) and B. (the first temporary registration number or the second temporary registration number and a card name described on the confidential number description card, and a data name declared beforehand on a card set column or a personal information column) respectively in regard to each constituent or the like possible by preparing a first check slip and a second check slip.SELECTED DRAWING: Figure 1
