PURPOSE: To simplify data registration to a health care card.

CONSTITUTION: On the screen of a touch panel display device 12, a doctor selects names of diseases or effects of medicies having a high frequency in use from a disease name data base 21 or a medicine effect data base 23 to preliminarily register them in a disease name table 22 or a medicine effect table 24. When the name of the disease of an examined patient or a the effect of a medicine will be registered, a health care card 17 is inserted to a reader/ writer 15, and a password is inputted, and the pertinent name of the disease or the pertinent effect of the medicine is selected from the disease name table 22 or the medicine effect table 24 displayed on the screen of the touch panel display device 12. A register button for the screen is tapped in this state to register the selected name of the disease or the selected effect of the medicine to the health care card 17.
