Abstract:

To form a document based on information on a card in a short time without making a mistake of entry, wrong characters and omitted characters.

This apparatus comprises a device body 30 in which a card information reading device 2 that optically reads information described on a surface of a card 1, a memory card reading/writing device 4 that reads or writes information from or into a memory card 3 and an input device 5 for inputting necessary information are housed and a printer 7 that prints a necessary item on a paper 6 in accordance with the information read by the card information reading device 2, read by the memory card reading/writing device 4 and inputted by the input device 5. Each of the above devices in the device body 30 and the printer 7 is so constituted that each is operated by a built-in power supply and an external power supply, respectively.
