A user transmits an event being the object of application and his or her employee's number as application information to a server 12 in order to make predetermined application. The server 12 specifies an application form necessary for the event by the event included in the application information and an application form information table 104. Furthermore, configuring items configuring the specified application form are specified by the specified application form and an item information table 106. Also, the employee information of the user is extracted from an employee DB 14 on the basis of the employee number included in the application information. Furthermore, necessary items to be inputted by the user are specified in order to prepare an application form necessary for the event designated by the user on the basis of the employee information and the configuring items. Then, one input form is prepared on the basis of the necessary items, and transmitted to a terminal device 10.
