A medical image processing device of this embodiment has a first image acquisition unit, a second image acquisition unit, a generation unit, and a calculation unit. The first image acquisition unit acquires a first fluoroscopic image of a patient. The second image acquisition unit acquires, from an imaging device that detects radiation with a detector and images the same, a second fluoroscopic image corresponding to radiation with which the patient is irradiated at a time different from that of the first fluoroscopic image. On the basis of the installation position of the detector, the generation unit generates a reconstruction image reproducing the second fluoroscopic image from the first fluoroscopic image that is virtually arranged in a three-dimensional space. The calculation unit derives, on the basis of the degree of similarity between the second fluoroscopic image and the reconstruction image, a suitable position of the first fluoroscopic image in the three-dimensional space. The generation unit generates the reconstruction image used by the calculation unit so as to have a range broader than the range corresponding to the second fluoroscopic image.
