Provided are: an optical element having superior optical characteristics and of which defects such as trapped air are prevented; and a method for forming same. The optical element has a first aspheric surface at one surface thereof. The first aspheric surface has a shape on the optical axis that is a concave surface, and at the outer periphery of the concave surface, the shape has a first extreme point that is a convexity. A first die (8) is formed having a first transfer surface (8a) having a transfer surface that is the reverse of the surface shape of the first aspheric surface of the optical element. Of the first transfer surface (8a), the central portion has a convex shape, and an extreme point transfer section (8c) that transfers/forms the extreme point has a concave shape. In a second die (10), a second transfer surface (10a) is formed that transfers the other surface of the optical element. A preform (18) is supplied onto the first transfer surface (8a), and is guided onto the extreme point transfer section (8c). The preform (18) is compressed/molded sandwiched between the gap of the first die (8) and the second die (10), thus obtaining the optical element.
